Veterinary statements: cleft palate or "no cleft palate"

Gracie is a five-year-old dark gray dilute tortie who was brought inside from the feral outdoor community by my father. She raised four kittens in 2016. She had just come into the house the day before the state's search and seizure to have a litter of kittens. She chose a less than ideal place, that is a half-tipped box under a desk. This was not going to be acceptable. I tried to move her box into the next room and put it near the nice warm wood stove. She didn't care for that. I tried moving the kittens into another box. No, didn't care for that either. I finally decided she needed to be confined and put them in the upstairs bathroom. Brought her a heater and turned it on. Brought her food with medication and water. Filled a pan with litter for her. I had already put several layers of clean towels into the box. Next I put a cover over the top to keep out the drafts. Gracie stayed in the box, so she decided this was an acceptable arrangement. Once they were settled, I left them alone to bond. That was also the day I had to dump all the litter boxes. Meanwhile I was still bottle feeding three kittens every three to four hours. I also had two cats due to have their bandages. I put off doing that until the next day, but the next day never came. In addition, I was treating quite a few cats with runny noses and eyes, including five kittens my father brought into the house. Doesn't it just figure, the next day is the one where I wake up to a spectacle, a circus had surrounded the place where I live and was encroaching on what was once and never will be again a safe place.

Gracie is incorrectly described as a black/orange tortie.
Gracie is incorrectly described as 1-3 years old. She is 5 years old at this time.
As described in the article Faulty Interpretations Lead to Wrong Conclusions, there are two copies of the 11/6/19 exam forms, one is incomplete and one is the original with the case number.
These are the original 11/6/19 exam forms.
You will see that Gracie's kittens were specifically checked for and found that none had cleft palate. By the way, for the person who filled out those forms, a pallet is something made of wood. Note also the instructions to weigh the kittens daily and supplement their feedings as needed.
So, how do they show up six days later at Falls Road with one dead of the consequences of cleft palate and all dehydrated?
